According to Figure 4-34, 'Wait for approval by manager' activity is 
before the decision for timeout versus approved. By looking at this picture, 
it seems that it will wait indefinitely, and only after getting an approval 
response, will go into evaluating the decision of the timeout. I think 
leaving out the 'Wait for approval by Manager' rectangle can be renamed to 
'Send request for Approval' to clarify this.
