A 4th column is described as storing dates in the caption, but no 4th column is shown
in the figure.

(73) Step 6 (directly under Figure 2-9);
Step 6 says to "Repeat steps 4 to 7...". I believe it should be steps 3 to 5. This is for 2006 (white cover) 
edition.
