1st line within table (underneath header "Property or value")

Seemingly, the first "Property or value" on the page is the "property" called "middle." It's seemingly a property because it is NOT INDENTED. However, it's acually an EXTENSION of the table on the previous page-pair; it's a VALUE of the property "vertical-align."

The best solution to this problem is to NOT BREAK THE "VERTICAL-ALIGN" PROPERTY ACROSS PAGES.

P.S. - As with ALL ONLINE FORMS, you should indicate that "a copy of this report will be sent to your e-mail address," or at least provide the option for that to be done. (Even if you WILL send a copy anyway, you should STATE that you will; otherwise, the user will imagine that the comment just might be lost in cyberspace with no way for the user to track it.)
