In Appendix B, "Free Bug Trackers" you omitted Flyspray (http://www.flyspray.org).

You can't list 'em all, however, I've been using Flyspray for a couple of years and though it is still(!) at a release less than 1.0, it is stable and solid. It is still actively being developed.

Flyspray is web based, LAMP problem tracking system. I provides for multiple projects, components, customizable status, role based priveleges and all the good things you'd expect a good PTS to have.
