(iii): changed the CA office's address to the list of cities

(xxx and xxxi): updated the CA office's address and changed all the
"ora.com"s to "oreilly.com"

(72): para. 3, line 5: changed "numbersr" to "numbers"

(83): para. 2, line 3: changed "LMI Erickson (a European
telecommunications company)" to "LM Ericsson"

(166): next-to-last para., line 1: changed "chose" to "choose"
(178): line 7: changed "Nobody knows me" to "Nobody knows my name"

{190}: in example 1, moved line starting with "Date..." so that it's
now line 2

lay-flat binding. If the page count exceeds RepKover's limit, perfect
binding is used.
