At the height of the dysfunction, when all the arrows are pointing downward and problems are not only apparent, they are sometimes front-page news, people begin to wonder: What went wrong? Sometimes the problems are evident long before they wreak havoc. While founders or leaders may have had innovative ideas and unique products, the culture can change to one that is no longer values-based.
