The lessons shared from previous LAPP incidents can be used, in part, to improve process safety performance in LAPPs. However, due to the nature of research and development, it is challenging to find incidents that apply to a specific laboratory or pilot plant. Thus, to make searching for pertinent incidents easier, this appendix contains details of and some of the findings.
Lessons shared from previous Laboratory and Pilot Plant (LAPP) incidents can be used, in part, to improve process safety performance in other LAPPs. Each incident case is arranged in the format shown in Table A‐1.
The case descriptions provide a brief overview of where, when, what, and why it happened. Since process safety and risk management system deficiencies are often the root causes of the incident, each case closes with a list of relevant CCPS Risk Based Process Safety (RBPS) management system(s) [7]. The framework of this model was shown in Table 1‐2. Had these management systems existed or been better implemented, they could have helped prevent or mitigate the consequences of the incident.
Note: This appendix should be used only as a tool to identify incidents with applicable learnings. Since these examples are not from an exhaustive literature search, they are not to be used for statistical or trending information.
