The console operator and I were reviewing his actions in response to a refinery wide power failure. Since he had three units under his span of control, he had three three-ring binders of emergency procedures in front of him. I asked him if he was supposed to do all the steps in one, then all the steps in the other, and so on. Or, was he to do the first step in each and then the second step in each? He responded with a smile and a shrug, “Your guess is as good as mine.” Unfortunately, that is about the response I expected.
