The TIQM Quality System is a comprehensive Quality Management System for
Total Information Quality Management based on proven Quality Management
Systems (QMS). The discrete processes of measuring IQ, measuring the Costs
of Poor Quality Information, and Improving Information Processes are not sus-
tainable without having a Culture and Habit of Continuous Information Process
Improvement. This chapter describes how to transform the organization’s culture
to establish a sustainable Total Information Quality Management Environment
that encourages everyone to take accountability for the Quality of Information
they produce, update, or apply in the performance of their Value Work.
The purpose of this chapter is twofold:
1. To outline how to establish an environment that leads to a culture of effec-
tive, Continuous Business and Information Process Improvement
2. To identify why TQM and IQM implementation initiatives fail and what
you can do to avoid failure

TIQM begins and continues with the umbrella Process of TIQM P6: “Establish
the Information Quality Environment.” This Process is not a discrete Process, but
