Investigations
Investigation is the process for obtaining facts and information for a better under-
standing of an event or to determine the cause of an event. Safety investigations
involve fact nding for the purpose of identifying safety breaches, loss, and safety
concerns. Compliance is required in 29 CFR §1904.0, “Recording or Reporting
a Work-Related Injury, Illness, or Fatality.” An investigation may expose activity
