Investigation is the process for obtaining facts and information for a better under-
standing of an event or to determine the cause of an event. Safety investigations
involve fact nding for the purpose of identifying safety breaches, loss, and safety
concerns. Compliance is required in 29 CFR §1904.0, “Recording or Reporting
a Work-Related Injury, Illness, or Fatality.” An investigation may expose activity
