In the early 1990s, the storage software engineering team at Digital Equipment Corporation developed a way to analyze and prioritize defect reports. That system and some early results of its application were described in a conference paper in 1995. At the same time, engineers at IBM were developing what has now become known as Orthogonal Defect Classification, which uses a very similar approach. The IBM approach has been described in several publications, and the implementation details have been documented in conference proceedings and on several websites.
