Chapter 12Managing Accountability

Every leader knows “holding people accountable” is an important part of managing safety performance. Successfully managing accountability begins by understanding what the term really means. Then it requires understanding the steps in the process to “hold someone accountable.”

Managing accountability is undertaken on the basis of a leader's understanding of the cause as to what went wrong, and what a follower did to contribute to the problem. It's a discussion with a follower lead by a leader taking place in the aftermath of a failure. It's a two‐way conversation intended to change future behavior for the better.

As to the process of holding a follower accountable, it is about asking the questions that need to be asked to establish ownership for what was done wrong, the consequences—actual and potential—for what went wrong, reaffirming expectations as to the follower's duties and responsibilities, and setting potential future consequences should behavior not change for the better.

The Five Ss provide the structure for your discussion to manage accountability. The first three S's—situation, significance, and specifics—set the stage for the core of the process: asking the questions that need to be asked. The natural place to start is by asking the person for “their side of the story,” and then, of course, digging deeper by asking Darn Good Questions.
