Whether it’s Bhopal, the BP Texas Refinery or the Fukushima nuclear power plant failure, common elements were present. There were groups of human beings making decisions facing uncertainty, a multitude of pressures for output, financial results and intergroup breakdowns in trust and communication. No doubt there was also cognitive bias that prevented them from seeing preventative measures. However, uncovering the emotional triggers created by our need to belong and past experience may be the most daunting challenge.
