Whether it’s Bhopal, the BP Texas Refinery or the Fukushima nuclear power plant failure, common elements were present. There were groups of human beings making decisions facing uncertainty, a multitude of pressures for output, financial results and intergroup breakdowns in trust and communication. No doubt there was also cognitive bias that prevented them from seeing preventative measures.
