ADD Workplace Questionnaire

The ADD Workplace Questionnaire has been developed to help you think about the effects of your ADD at work in a systematic fashion. This questionnaire can be used by your therapist or counselor to help you consider issues of workplace functioning. It may be helpful to use this questionnaire with a career counselor if you are in the process of career change.
