What Are the Expectations for the EHR to Perform Measurement?

The current process for measuring quality using EHRs is evolving. Most existing measures have not been written in a format to allow direct queries to EHRs to extract data. Much of the information needed to evaluate these existing measures is based on information from claims submitted for payment or from human abstractors who review medical records in detail to know whether the measure criteria are met.
