Questions by Subareas
You have already identified the areas of each dimension you need to question, and you have a guideline as to the number of questions per dimension. Figure 7-4 features a Question Development Form. Like the other forms introduced in this book, it is on the accompanying disk.
The Question Development Form is to be used for each dimension subarea and for each dimension in which you did not identify a subarea to survey. You begin by completing the top of the form. For example, if the dimension is Facilities, the subarea is Environment, and the number of questions guideline is 6 (total per dimension divided by the number of subareas in the dimension), the completed top of the form would be:
