This book is deliberately oversized to allow the included forms to be usable as well as useful. I invite you to use any of my own contracts or agreements as your own; and I hope you’ll have reason to do so. I’ve tried not to clutter this Appendix with unnecessary facts and figures and have included only material that I hope will help you achieve success in whatever type of licensing endeavor you’ve embarked on.
Although every effort has been made to double-check all phone numbers and addresses, I know it’s inevitable that some will become obsolete as time passes. In the publishing world, many months can pass between the time a manuscript is prepared and the printed book appears in stores—and of course more months or years can pass before the book winds up in any one reader’s hands. Some of the websites that have been recommended might have vanished, some inventor clubs closed or moved, and some forms may have been replaced by new ones. If any of this becomes the case and causes a measure of inconvenience, I apologize and ask for your understanding.
