Developments in engineering and the adoption of safety management practices within the chemical and process industries have meant that both system, and health and safety performance are generally improving. But there are still accidents and incidents and processes do not run smoothly all the time. This is often because the human element in the system has not been designed well enough or work practices have not been sufficiently considered.
