Review of common human vulnerabilities and contributions to past accidents

Abstract

Developments in engineering and the adoption of safety management practices within the chemical and process industries have meant that both system, and health and safety performance are generally improving. But there are still accidents and incidents and processes do not run smoothly all the time. This is often because the human element in the system has not been designed well enough or work practices have not been sufficiently considered. This chapter presents a review of past incidents to illustrate the typical human vulnerabilities in work systems that can precede and contribute to a major accident.
