The manager of the maintenance team is accountable for the quality of the system after it is transferred to the maintenance team. The initial quality is a reflection on the project team responsible for the implementation; but as time goes on, the quality will be a reflection of the maintenance team, and rightfully so. The maintenance team will continue to apply enhancements and defect fixes to the systems in order to improve the systems. But there is also the possibility of inadvertently introducing new defects into production along with the enhancements and defect fixes.
