In today’s world safety has two faces: real safety and documentation of that safety. First, “is one safe?” and second, “can we prove through documentation that we are safe?”

Going without an incident is not proof that one is safe; it might be because the users and institution have the correct polices and follow them. It could be no one has fallen into the pit that is in front of them, just walking around it. This is where safety documentation becomes very important.
