REAL-WORLD EXAMPLES OF THE IMPACTS OF POOR DATA

The following brief examples show how poor data quality has negative implications (financial, reputational and other) that are sometimes highly significant.

• A woman bled to death in 2015 after a spelling mistake meant blood intended for her during an operation was sent back. The woman concerned had an unusual name, which had been mis-spelled on one form.
