D. Sample Evaluation Templates

Head-of-School Evaluation 1 (Johnson 1986: 45 to 49)

This form was developed by a metropolitan coeducational Friends day school of 1,000 students from age four through grade 12. The same form has been used with equal effectiveness by a small elementary school, demonstrating its wide applicability.

Directions: Read the entire questionnaire before answering any questions. Please note that you are asked to grade only at the end of each section. With regard to Section 6, “Personal Characteristics,” if you have any areas of concern, please mark them with a C. If you have no opinion or knowledge of certain characteristics, mark them with an N.

Grade the head according to this scale:

5 = Consistently outstanding

4 = ...
