This form is based on the excellent one produced in 1967 by the Association of Governing Boards and Universities, in Washington, D.C. The form shown here is a modification of the AGB form done by a state association of independent schools to make it apply more directly to schools; it is filled out by board members or anyone else concerned with evaluating the board’s performance.
