Incident investigation is an important topic in process safety. In a general sense, incident investigation in process safety, like any kind of retrospective investigation, helps shine light into events that already occurred and searches for comprehensive information of combinations of separate events leading to incidents to identify what, when, why, and how an event happened. This “what went wrong” information learned from process safety incident investigation plays a much broader and more important role in shaping future action to prevent the same and similar things from happening.
