Safety

Safety has largely been addressed in this textbook by specifying valve characteristics (fail-open, fail-closed), redundancy (multiple pumps, bypass valves, sight glass vs. pressure difference for level sensing) and vessel pressure relief valves. Major disasters (chemical or otherwise) usually result from multiple bad decisions by humans “in the loop” and not just simply one bad decision.

After studying this module, the reader should be able to

  • Understand the major explanation for the Bhopal accident.

  • Understand the important role that humans had in the BP Texas City disaster.

  • Discuss the decisions made by Boeing engineers that impacted the 737 Max 8.

  • Understand that disasters usually occur due to a combination of factors.
