What is it?

THE CHANGE MANAGEMENT PLAN DEFINES what you need to do to achieve the change management strategy. The plan identifies the order and dependency of tasks and activities and the resources required to deliver them. The plan also provides the structure so that work streams for each of the lenses in the change prism are integrated across the phases and tasks/activities are aligned to the overall project plan.
