Let’s assume at this point that the complete Root Cause Analysis (RCA) process has been followed to the letter. We have conducted our modified Failure Modes and Effects Analysis (FMEA) and determined our “Significant Few.” We have chosen a specific significant event and proceeded through the PROACT process. An identified RCA team has undertaken an organized data collection effort. The team’s charter and Critical Success Factors (CSFs) have been determined, and a Principal Analyst (PA) has been named.
