Appendix A. Quick Reference Guide
If you are faced with an emergency situation and do not wish to thumb through this book to figure out what to do, here is an outline of immediate actions you should take.
Interview all participants and witnesses to the incident and learn the who, what, when, where, and why of the situation.
Have all participants and witnesses put their statements in writing. Have them sign off on their written statements.
If the witnesses will not write out their statements themselves, you should do it instead and then ask them to review the statement for accuracy. After the witness agrees and any necessary corrections have been made ask them to sign off on each page to signify agreement with its accuracy.
