Prevention data can be gathered in any phase of the product cycle. The length of time between the introduction of a defect, its discovery, and its repair may vary dramatically depending on the phase in which the defect is introduced. Defects introduced in the design phase may not be discovered until the product is released to users and may not be repaired until a future release.
