APPENDIX ALeadership Change-Risk Diagnostic Instrument

This Leadership Change-Risk Diagnostic Instrument is designed to assist public and nonprofit sector leaders/managers as they plan a major change or transformation effort. The instrument considers aspects of change risk and organizational capacity, examining how “pertinent” a particular risk or capacity is to the change effort versus “organizational change preparedness or performance.” Measuring pertinence assists a change leader in identifying the extent to which different aspects of the organization and its environment are important to the change effort (i.e., the aspects that may influence the success of the change or transformation effort).
