Escalating once a threshold is reached

We know how to perform an action if a threshold is reached, such as the temperature being too high, the available disk space being too low, or a web server not working. We can send a message, open a ticket in a tracker, run a custom script, or execute a command on a remote machine. But all of these are simple if-then sequences; if it's this problem, then do this. Quite often, the severity of the problem depends on how long the problem persists.
