Menu item 2. details the steps to create a certificate chain consisting of interaction with the CA.sh shell script. There is a line beginning with the text: 'Next, create a certificate request; this will generate an unsigned...' It is formatted to appear as output from the CA.sh script but it should be step number 3, which would make Step 3 on page 209 Step 4.

With the SSL module, there is an Apache directive SSLPassPhraseDialog that allows the apache server to run a scripts that outputs the SSL passphrase so that the server can startup automatically. AUTHOR: The SSLPassPhraseDialog option will cause mod_ssl to query the administrator for a pass phrase.
