you leave ellipsis for the #customControlheader and customControlfooter parameters. I have no idea on what the complete list should be.

and

customControlHeader does not appear to work.

The Jira documentation states:

#controlHeader ($action $customField.id $customField.name $fieldLayoutItem.required $displayParameters.noHeader)

This works, your example does not.

Note from the Author or Editor:
Those header lines are hard to fit in the width of a page. I'll do something about it for the next update though, even if it's just a warning. The code samples have the complete line.
