It appears that figure 3-14 is showing a repeat of the top section of figure 3-13, with the termination resistance at the driveN end of the transmission line (B). Instead, as I understand the text referring to figure 3-14, it should show termination resistance at the driveR end (A), and perhaps the capacitor mentioned.

The changes discussed in the text are correct about circuit change options. The diagram does not reflect those clearly. This will get updated in the next edition.

Should be "(due to C to D and Z2)"?

It appears to me that the voltage source connected to the Gate in Case E is drawn in reverse. Based on the circuit's intended function to lower the Source voltage (V_S = V_Pwr), the positive terminal of the voltage source should be connected to V_Pwr, and the negative terminal should be connected to the Gate.

The accompanying text on page 271 states:
"Case E shows a situation where the transistor is on. The voltage at the gate has been lowered so that V_GS has exceeded the threshold voltage."

Since the MOSFET in question is a P-channel, It might be clearer to phrase it as:
"Case E shows a situation where the transistor is on. The voltage at the gate has been lowered (made more negative from previous case, where V_GS was 0V) so that V_GS has exceeded the (negative) threshold voltage."
