block on the top left

On the opening picture to chapter 5, it reads: "Reations often block ability to think systematically".
There is a "c" missing. It should be: "Reactions often block ability to think systematically".

Practice 3: Learning Outcomes (last paragraph)

It says: "Select a few skills on this list ..."
It wasn't clear to me what "this list" was supposed to be. You state that you have given questions and examples to consider (past tense). The only relevant list I could see was the one provided above. The questions and examples for this list, however, are provided after Practice 3. So, the past tense without indication that the questions and examples are yet to come seems confusing to me, especially since previous practices were contained under one single subheading. I would indicate clearly that the reader should continue reading before doing the exercise (if that is the intention) or point to a specific previous list (otherwise). I would probably write something like "Select a few skills from the list provided above. Explanations, questions and examples for each item are provided below."

5th paragraph

It says: "this is a thinking-in systems-activity"
I suppose it should be: "this is a thinking-in-systems activity"

2nd paragraph

It reads: "... the internal thinking patterns they'd learned ... in the old paradigm ... did not translate well into the new one paradigm."

I suppose it should be "the new one" or "the new paradigm" instead of "the new one paradigm."

Top-right corner

It says "Success for who?" when it should be "Success for whom?"
Notably, it is correct in the first paragraph of "Successful Systems Equalize Impact" on page 223
