This appendix provides samples of two pre-site survey documents that will help you create your own pre-site survey template, which you learned about in Chapter 6, and a sample site survey report, which you learned about in Chapter 13.
Pre-Site Survey Forms
The first form was provided by a professional site survey company, NeTeam Corporation. Some readers may be surprised by a few items in this document, but in many cases items such as drug testing or insurance certificates can delay any work to be performed until these items are completed.
