  1. AssessmentWho are you now?

  2. VisionWho do you want to be?

  3. Overcoming barriersWhat are the obstacles that stand in your way, and how can they be overcome?

  4. ActionWhat are the specific steps needed to move toward your vision?

  5. SustainingHow will you deal with setbacks and maintain momentum?
