In Chapter 5, we developed and illustrated a four fold classification of the causes of technological disasters: technical design factors, human factors, organizational systems factors, and socio-cultural factors. In Chapter 6, we undertook an historical analysis of the three industrial revolutions. These conceptual frameworks will now be combined into a 3 × 4 matrix in an effort to provide a more detailed explanation of technological disasters (see Figure 7–1).
