THE MODEL PERFORMANCE APPRAISAL TEMPLATE that follows can be used right off the shelf or modified using formats and content found throughout the book and in the six sample templates that follow in Appendix D. Feel free to use this as a starting point but to amend it depending on your company’s stage of development, culture, and communication style.
