What Goes into the Evaluation Plan?
The evaluation plan should identify what or whom to evaluate, how often and when evaluations will occur, who conducts the evaluations, who compiles the data, what data is collected and how often, how the data is used, what evaluation instruments or forms are needed and who develops them, who reports the results to whom and how often, and what happens if evaluations are unacceptable.
