If I haven't eaten enough, I tend to get distracted and start to think because I'm hungry. It might not be thoughts about my stomach, but I wouldn't be thinking if I were full – I'd be focused. This can also happen with sleep, illness, or any sort of body problem.
It's not as common as the "understanding" problem from above, so first always look for something you didn't fully understand. If you're really sure you understood everything, then physical problems could be a candidate.
