Safety and reliability are related but different key characteristics of a high performing clinical microsystem. Safety is providing care and services in a manner that minimizes the risk of avoidable harms. Reliability is doing the right thing at the right time in the right place every time. Evidence-based care can tell us what is right but often there is no evidence-base to tell us what is right.
