I am utterly amazed to find myself in this position! Writing a book takes a great deal of enthusiasm, together with the encouragement and skill of a surprisingly dedicated team of people. That I was asked to produce a second edition of this book showed that I had not tried sufficiently hard enough to upset them the first time around. Asking me for a third edition smacks of real staying power on their part. I would also like to thank Jon Jagger who co-authored the first edition of the book with me, and who remains the inspiration behind many of the exercises in the early sections in this book.
