Chapter 10, Sections 10.2.3 and 10.2.4 used the concept of gross disproportioinality factor (GDF), as a multiplier to inflate the cost per life saved criteria according to the margin between the predicted risk and the maximum tolerable risk.
As was explained in Chapter 10, common practice is to assess the GDF on a logarithmic scale between x10 at the maximum tolerable risk down to 2 at the broadly acceptable level.
The spreadsheet (on the next page) performs this calculation. The cells are as follows:
