Tom started my interest in this area many years ago, and Dan helped deepen it.

I'd like to thank McGill University and my colleagues there for putting up with my mysterious absences. I'm not sure everybody believed I was “working on my book.” Special acknowledgment to my friend Lise for the scones (and general brilliance) and to Mike for being solid and fun, sometimes simultaneously.
