Cover sheet
The cover sheet might not seem too important, but it does set the tone for the entire report. It is the first thing the stakeholders will see, so it needs to look good. It should have the appropriate logos on it, both yours and the company you're running the tests against. Make sure that the logos are sized correctly and look good, not just something that you cut out from their website. Marketing yourself correctly will not only help grow your individual brand but also draw the stakeholders into wanting to read the report.
Pertinent information such as a high-level name of the work that was performed. For example, was it "Penetration Testing of Corporation ABC Headquarters" or was it something more like "Web Application Report ...
