Basically you create custom extensions for and with Visual Studio taking advantage of specific project templates. To enable Visual Studio 2010 extensibility projects, you need to download and install the Visual Studio 2010 Software Development Kit, which is available from the Visual Studio Extensibility Center located here: http://msdn.microsoft.com/enus/vsx/default.aspx.
