The quarterly results looked excellent. One operations team reported 92% AI utilization, efficiency gains were on track, and team satisfaction surveys suggested strong acceptance. The operations director had every reason to feel confident about sustainable transformation success.
But alignment was quietly deteriorating.
Six months later, override rates had climbed from 23% to 67%, manual verification processes had returned despite AI availability, and experienced team members were privately advising new hires to “check everything twice.” The organization had stopped performing active alignment maintenance, assuming that initial success would sustain itself through momentum and habit formation.
