The EQ-5D questionnaire
The EQ-5D (Kind et al., 2005) is a short questionnaire primarily designed for
self-completion by respondents. Thus, it is particularly effective when used in
postal surveys, clinics or face-to-face interviews. The questionnaire consists of
two parts: the first one, shown in Figure 1.3, is a descriptive system about the
health dimensions considered as most relevant. These are mobility, self-care,
usual activities, pain/discomfort and anxiety/depression. In the most recent
version, each dimension has 5 levels: “no problems,” “slight problems,” “mod-
erate problems,” “severe problems”
