Foreword
Physicians are trained according to the time-honored principles that underlie the
Oath of Hippocrates: curing the patient, when possible, and alleviating pain and
suffering, while “doing no harm.” Whether medical students were taught years ago,
when I was trained, or today, in an interactive environment with problem-based
learning and a vertically-integrated curriculum, the emphasis has been on maxi-
mizing one-on-one interactions between physician and patient. e development
